New Client Request

You can view my availability and book an appointment for a 55-60 minute Individual Session or 15 minute Consultation at the links below. 

If you are a new client you MUST request an appointment at least 3-4 days in advance of the appointment date to allow for time to get your billing set up. Please also fill out the forms from CarePatron so that I can load you into a platform for financial processing based on your insurance/private pay needs. You MUST complete all paperwork in CarePatron at least 3 days prior to our first appointment.

Please keep to our agreed upon scheduling--weekly, biweekly, or monthly--unless there is a reason for an earlier appointment. Remember in a crisis situation to use 988.
